Cancellation Policy

4Quest Biofeedback Booking Policy: To ensure a smooth and efficient experience for all clients, please review our IV Therapy booking policy below: 1. Appointment Scheduling: 4Quest Biofeedback appointments must be scheduled in advance through our website, phone, or in person. (CALL OR TEXT: 720-815-9303) A valid credit card is required to reserve your appointment. 2. New Clients: New clients must complete an intake form and a brief health screening before receiving 4Quest Biofeedback. A consultation with our provider may be required based on your health history. 3. Cancellations & Rescheduling: Cancellations or reschedules must be made at least 24 hours in advance to avoid a cancellation fee. Appointments canceled with less than 24 hours’ notice or missed without notice will incur a $50 fee. 4. Late Arrivals Please arrive on time. If you are more than 10 minutes late, your appointment may need to be shortened or rescheduled, and a fee may apply. 5. Payment: Payment is due at the time of service. Packages, memberships, and promotions must be paid in full in advance and are non-refundable. 6. Medical Eligibility We reserve the right to decline or defer 4Quest Biofeedback services if there are any contraindications or safety concerns. All clients must be medically cleared by our provider before treatment. 4Quest Biofeedback duration may vary per client from 30 minutes to 4 hours.
