Cancellation Policy

Cancellation & No-Show Policy We require 24 hours’ notice for cancellations or rescheduling. Appointments canceled within 24 hours or missed will incur a minimum $50 fee. Prescription Refills & Form Fees A $25 administrative fee applies to prescription refills and form requests made outside of an appointment (e.g., FMLA, disability, school/sports forms, prior authorizations). Fees apply per request and must be paid before processing. • Refills requested during a visit are included at no charge • Refills within 2 weeks of a visit are free • If not seen within 6 months, an appointment may be required • Please allow 5–7 business days for completion Prior Authorizations A $25 fee applies to each prior authorization submission, regardless of approval outcome. Supplement Return Policy Returns accepted within 15 days if unopened, sealed, and with receipt. • No returns on special order items • Refunds issued to original payment method Credit Card on File A valid card is required on file. You authorize us to securely store and charge your card for balances including copays, deductibles, no-show fees, and uncovered services after notification.
